Residents at Uplands House benefit from a clear progression route if appropriate. Our step-down model enables people to move from a higher level of support in the main house to more independent living in one of our self-contained flatlets, helping them build confidence, skills, and autonomy at a pace that suits them.

We welcome adults aged 18 and over, including those with co-existing physical health conditions. Each resident receives a carefully planned package of care shaped by their own choices, needs, and life goals. From the very first day, residents are encouraged to be active partners in their care planning and that can include input from families and care and be invited to be part of the journey.

At Uplands House, we believe in creating more than just a place to live—we aim to create a home, a place to grow and thrive in and to look forward to the future with confidence.

MAKE A REFERRAL

Referrals Process for Professionals & Self Funders

To initiate a referral, please submit the following documentation via email to Referrals.Specialist@caretech-uk.com

  • The individual’s most recent Care Act Needs Assessment
  • Support Plan
  • Relevant clinical or behavioural reports
  • Any recent incident or behavioural analysis
  • Additional documentation providing a comprehensive overview of the individual’s current needs

Upon receipt, the referral will be reviewed by the Registered Manager and the Multidisciplinary Team (MDT). The MDT will determine the most appropriate professionals to carry out the assessment. Typically, two assessors from different disciplines (e.g., a nurse and a psychologist) will be assigned, particularly where the individual presents with complex clinical or psychological needs.

If the individual appears potentially suitable for the service, an assessment will be arranged. This may begin with an initial video consultation or a face-to-face meeting. We will liaise with the current placement to coordinate a suitable time for the assessment, ensuring key members of the existing care team are available to contribute.

Assessment Timelines:

  • Standard assessments are conducted within 3 working days of receiving a complete referral.
  • For urgent referrals requiring placement within 48 hours, please clearly indicate this in your initial contact to enable prioritised processing and assessment.

Following completion of the assessment, the full MDT will convene to evaluate the individual’s needs and determine suitability for placement. Where appropriate, further information may be requested, or an alternative service within the CareTech/Coveberry portfolio may be recommended.

The Service Manager or Business Development Manager will promptly inform the referrer of the MDT’s decision. If the individual is deemed suitable:

  • A comprehensive Assessment Report and Care Summary will be provided within 3 working days
  • A formal placement offer will be issued, including proposed costs and supporting documentation

To accept the offer, please confirm by email, including:

  • Agreement to the proposed weekly cost
  • Full invoicing details for all relevant funding bodies

If the referral is declined, the Service Manager or Business Development Manager will contact the referrer directly (via phone or email) with a clear rationale and, where appropriate, suggest alternative care pathways or services.

Post-Funding Agreement:
Once funding has been confirmed, we will coordinate any necessary discharge or transition planning and agree an admission date.

Urgent Referrals
We offer a fast-track admission pathway for individuals experiencing acute behavioural deterioration or cognitive decline requiring immediate intervention. For urgent referrals:

  • Contact the Referrals Team directly
  • Clearly state the referral is urgent

An assessment will be arranged within 24 hours, or sooner if clinically indicated.
